This demo is setup with following criteria:

Products are marked as bulky in their product listing. It is possible to set up lots of shipping groups (1 per product if required!). So for example if you could have groups for SMALL, MEDIUM, LARGE products and ship based on these, or even do things like group according to category.
To understand how this works I recommend you open the csv file and then add some products and check the rates using the estimator on the checkout page.
This is working on a "Per Package Totalling" - where it finds the totals for each shipping group then adds them together. Other options are available including highest shipping group value, per item (where rate is multipled by #items, etc).
